Remote Patient Monitoring (RPM) Service – Terms and Conditions
Please read these Terms and Conditions ("Terms") carefully before using the Remote Patient Monitoring (“RPM”) services provided by [iSpin Health LLC]
By enrolling in our RPM services, you (“you” or “the patient”) agree to comply with and be bound by these Terms.
1. Service Description
Our RPM services involve the use of digital technologies to collect your medical data (such as blood pressure, glucose levels, weight, or heart rate) from you in one location and electronically transmit that information to healthcare providers for assessment and recommendations.
This service may include:
Provision of FDA-approved monitoring devices
24/7 data transmission and storage
Daily or periodic monitoring by licensed healthcare professionals
Clinical interventions when necessary
Communication via phone, email, or secure portal
2. Eligibility, by continuing to use iSpin Services, you agree to the following:
You must:
Be under the care of a provider who has ordered RPM services for you.
Have one or more chronic or acute medical conditions that require monitoring.
Be able to use the monitoring devices and transmit data as instructed.
Be at least 18 years of age or have been legally emancipated.
You understand and agree that these Terms are a legally binding agreement and the equivalent of a signed, written contract.
You will use the Services in a manner consistent with applicable laws and regulations and these Terms, as they may be amended by iSpin Health, LLC from time to time.
You understand, accept, and have received these Terms and the Patient User Privacy Policy, and acknowledge and demonstrate that You can access these Terms and the Patient User Privacy Policy at will.
3. Use of Equipment
You may be provided with medical devices (e.g., blood pressure monitor, scale, glucose meter).
These devices are to be used only by you and only for the intended medical monitoring.
You agree to take reasonable care of the equipment and return it if requested.
Lost or damaged devices may result in a replacement fee.
4. Data Collection and Privacy
We collect and use your health data in accordance with our Privacy Policy. All information is stored securely and in compliance with HIPAA regulations.
You consent to the collection, use, and sharing of your health data for the purpose of providing RPM services, improving treatment, and fulfilling legal and regulatory requirements.
5. Fees and Insurance
RPM services may be covered by Medicare, Medicaid, and certain private insurance plans.
You may be responsible for co-pays, co-insurance, or deductibles depending on your insurance plan.
You are responsible for informing us of any changes to your insurance.
If your insurance does not cover RPM services, you may be billed directly.
6. Disclaimers and Limitations
RPM is not a substitute for emergency care. In case of a medical emergency, call 911.
The service is intended to supplement—not replace—in-person visits and traditional healthcare.
We do not guarantee that RPM services will prevent health complications.
7. Termination of Service
RPM services may be terminated by:
You, at any time, with written notice.
Us, if you are non-compliant, fail to transmit data, or are no longer eligible.
Your healthcare provider, if RPM is no longer medically necessary.
8. Amendments
We reserve the right to update these Terms. Any changes will be communicated in writing or via our patient portal. Continued use of the service after updates constitutes acceptance of the new terms.
